Health Sciences Doctoral Program Capstone Project

NOTE: In addition to the procedures outlined below, you must observe any regulations approved in your program that are not precluded by these instructions. I. General Information and Requirements

All students who were advanced to Level 2 and who have completed all required coursework must submit to the Office of Sponsored Research (Room 8309) a copy of the Human Participants Clearance Form (Form A) and the Project Proposal Clearance: Human Participants (Form B). Form B must be signed by the student and the Dissertation Advisor. It will then require the signature of the Chair of the GC Committee on the Protection of Human Subjects.

The doctoral degree requirements are considered met on the date the coursework including clinical affiliations/residency is completed,*  the capstone project is actually deposited in the Mina Rees Library, the Registrar clears the student's record, and all fees are paid. Students who are delinquent in their accounts with any division of the University will not be granted the degree.

*Note: May or may not apply to MA to Au.D. students depending on their program of study

Specific instructions for the preparation and deposit of the dissertation will be found in the following sections. The Dissertation Assistant may refuse to receive the dissertation or capstone project if it does not conform to the established standards. While the dissertation or capstone project may be deposited at any time during the year, for the June 2010 commencement, deposits must be completed by March 19, 2010.

In order to allow sufficient time after your project is approved for preparing the capstone project for deposit and to obtain the necessary clearances, your presentation should be scheduled at least five (5) business days prior to your anticipated deposit date.

Degrees are awarded only on September 30, February 1, or on the date of The Graduate Center's annual commencement generally scheduled for late May or early June. Diplomas will be prepared for distribution only at commencement. After depositing the capstone project, students eligible for graduation will receive from the Registrar an interim certificate testifying to completion of the degree requirements. You will receive information about commencement in April. If you have any questions that are not answered in these instructions, consult the Dissertation Assistant, in Room 2304 (2nd floor) in the Mina Rees Library (212-817-7069).

III. Preparation of Deposit Copies –

A.    Print Copies

Approval Page:

The approval page carries two required signatures, those of the chair of the Examining Committee and of the Executive Officer. It also lists names (in type) of the Co-Directors of the Program (see sample approval page). The signatures must appear on the approval pages in both print copies of the dissertation, although the page in the second copy may be a photocopied reproduction of the signed page in the first copy. Candidates are reminded that the paper requirement for the print deposit copies also applies to the approval page.

 

Corrections and Insertions:

 

No ink corrections, white correction fluid or insertions between lines on final print deposit copies.

Fonts

Any legible font except script, italic, or ornamental fonts equivalent in scale to 10pt. Arial or 12pt. Times New Roman. Italicized font may be used for non-English words and quotations. Applies to all text including captions, footnotes/endnotes, citations, etc.

 

Line spacing

Double-space: abstract, dedication, acknowledgements, table of contents, and body of the manuscript, except for quotations as paragraphs, captions, items in tables, lists, graphs, charts. Single-space: footnotes/endnotes, bibliographic entries, lists in appendices.

 

Margins

Left: 1 ½"; Right: 1"; Top: 1"; Bottom: 1". ALL material, including figures, headers/footers, footnotes/endnotes, and full-page images must appear within the margins of the manuscript. Page numbers are the only exception; these must appear at least ¾" from the edge of the page, but do not need to appear within the margins.

 

Oversize materials

Acceptable. Fold maps and other material larger than 8 ½" x 11" to manuscript size. Large digital files such as PowerPoint presentations may be printed out and included with the dissertation OR submitted on a dvd which will be stored in a pocket on the inside back cover of the bound dissertation copies.

Paper for print deposit copies:

White bond paper, 8.5 inches x 11 inches, unlined, at least 20-pound weight and 25% rag or cotton content is required for all deposit copies. No hole punches

 

Print size and quality

Laser printed copy with crisp, dark black characters; single-side printing only

 

Quotations:

The doctrine of “fair use” applies [http://library.gc.cuny.edu/copyright/fair_use.html]. UMI suggests that for a quotation or quotations totaling 150 words or more, the copyright owner's permission should be requested. If other forms of copyrighted materials (photographs, charts, tables, etc.) are included in the dissertation, permission must be obtained for their use. (If permission letters are not supplied, the copyrighted materials may not be published by UMI.) Reproduction Processes

The two print deposit copies of the dissertation may be the original + one photocopy. Photocopies must be good-quality reproductions, laser-printing preferred.

Students enrolled in Health Sciences Doctoral Programs must use:
Publication Manual of the American Psychological Association (5th Edition, Washington, D.C., APA, 2001). [Reference BF 76.7 P83 2001]

Title Page:

The title page must include the full capstone project title, the complete name of the author, which should match the Registrar's record of your name (any official changes of the author's name must be made by submission of a legal document to the Registrar's Office), the capstone project statement, and the year of the degree [Sample Title Pages]. If the title includes formulas, symbols, superscripts, subscripts, Greek letters, or other non-alphabetical symbols, you should use word substitutes. Such symbols, while prohibited from being used in the title, may be used throughout the text of the capstone project.

 

 

  B.    You must also deposit an electronic version of your PowerPoint presentation (Applies to Au.D. students) and a pdf version of the text portion of your dissertation on a disc along with two paper copies.

 

 

IV. Deposit Procedures

A.    As soon as your defense date is set make an appointment to meet with the Dissertation Assistant to deposit your project. Contact: Judy Waldman, Dissertation Assistant, Room 2304, Mina Rees Library, Graduate Center, 212 817-7069

B. Complete the following surveys and clearance form online. Print and bring them to the deposit meeting. DO NOT EMAIL ANY OF THE FORMS.:

    1. NSF Survey (Note: brochures are available that describe the survey and provide information about your the confidentiality of the information gathered).
    2. Graduate School Exit Survey
    3. Deposit Clearance Form

C.    You are required to bring the following items to the deposit meeting:

    1. Completed NSF Survey, Exit Survey and Deposit Clearance form
    2. Two official print copies of your dissertation with signed approval pages
    3. Disc containing a pdf file of your capstone project including your PowerPoint presentation if applicable
    4. Cash/Check or credit card (not recommended) for payment at the Bursar's Office
    5. Signed dissertation approval page. The approval page carries two required signatures:

      • the signature of the CHAIR of the Examining Committee (e.g. ADVISOR)
      • the signature of the Executive Officer.

    It also lists the typed names of the Co-Directors of the Program. Signature are not necessary for the Co-Directors. (see sample approval page)

D.    Students and Advisors are responsible for reviewing the accuracy of their capstone project copies prior to submission. The final copies must be free from all typographical errors and correct in every detail of style and format. Print copies must be unbound, un-punched, and in typing-paper boxes or in envelopes.

 

E.    The two print deposit copies will be bound and distributed respectively to the Reference section of the Mina Rees Library and to the Executive Officer of your program. If you have received the degree as a recipient of a federal award, you should inquire about the necessity of providing the agency with an extra copy of your capstone project.

 

F.    When the capstone project is deposited, the fees stated in Section V will be assessed.

 

G.    Holders of a Perkins or Federal Direct Student Loan must arrange for an exit interview with the Office of Financial Aid. You must give the Office of Financial Aid (212 817- 7460) one week's notice.
